REQUEST FOR QUALIFICATIONS
FOR POTENTIAL TEAMING OPPORTUNITIES
- Firm background/history. Please include the number of years your firm has been in business and a listing of all services your firm offers. Provide addresses, phone and fax numbers for all offices. Indicate who will be our primary contact and give a phone and e-mail address for that person.
- Provide a staff breakdown, indicating the number of staff members in each discipline you offer. Please separately indicate the number of professionals registered in the state of Texas.
- Provide a current resume for each professional project manager who might be assigned to projects with Command Commissioning. Resumes should include:
- Educational background (type of degree, field of study, university, year graduated)
- How long the individual has been with your firm
- A list of specific projects on which the individual has worked
- Texas registration number, if applicable
- List at least 10 projects completed within the last 5 years. Please provide the following information on each project:
- Description of the project
- Year completed
- Construction cost and/or design fee
- Services your firm provided
- Firm's role (prime or subconsultant)
- Client contact and a current phone number.
- If you are a registered HUB, M/WBE, or SBA 8(a) firm, please provide a copy of your certification form.
- In addition to the references provided in your response to item 4, please provide three additional client references.
Reference Inquiries
The following questions are representative of the types of questions that might be asked of the individuals you have listed as references.
- Did the firm meet or exceed commonly accepted standards of good practice?
- Was the firm's work conducted in a timely manner, keeping to schedules and deadlines?
- Did the firm provide value to the team in terms of offering cost-saving measures or innovative design ideas?
- Were invoices provided in the manner outlined in your contract?
- Were they timely?
- Were they consistently accurate?
- Would you recommend this firm to others?
